The goal of Healing Hearts Home Health is to assist patients in their homes by enabling them to live a safe, healthy, and happy lifestyle. We encourage the highest level of patient independence. Home health provides professional nursing care and therapies on an intermittent basis. Both medical and non-medical services are provided by registered nurses, certified nursing assistants, physical, occupational, and speech therapists, and medical social services. Your personalized treatment plan will be established with your physician and implemented by our compassionate team members. We are here to promote health, prevent disease, provide rehabilitation, and care for the sick.

How it works
Home Health Admissions Process

Home health is a cost-effective option for receiving health care services. Returning to one’s home and family can promote quicker recovery and improve the quality of life for both patient and family caregiver.

We accept all insurance plans. You may already qualify for free services based on your medicare coverage call us now.

  • 1. Your doctor will prescribe home health
  • 2. Our compassionate staff will meet with you and your family to review home health needs and to establish start of care
  • 3. Healing Hearts Home Health will confirm insurance coverage
  • 4. A Healing Hearts Home Health nurse or therapist will individualize your plan of care
  • 5. You and our staff will set up the number and frequency of visits based on your individual needs.
  • 6. Healing Hearts Home Health will confirm Medicare eligibility, insurance coverage, or will review payment options.

Medicare Coverage
Medicare typically pays 100% for home health services when all conditions are met:
  • Patient is Medicare eligible or confirmation of alternate pay source eligibility
  • Meets homebound status
  • Plan of Care (485) established with you and your physician
  • Physician’s orders received by the agency
  • Documentation of all services throughout the process
  • Care is reasonable and necessary, and intermittent or part-time

Home Health Aide Services

Home health aides are trained to help with a variety of tasks to assist the patient to function independently and safely in their homes. They work with skilled professionals such as nurses and therapists to ensure the patient can achieve activities of daily living safely.

Medical Social Worker Services

Medical Social Workers assist patients and their families in adapting to illness as well as providing information on how to use the community resources available to them. Medical social workers can help patients and family members develop coping strategies, assess patient needs, assist in the development of treament plans, and provide emotional and mental assessments on the patient. Medical social workers can also offer bereavement counseling, and assist with patient discharge.

Therapy Services

Treating patients in their own home allows physical therapists, occupational therapists, and speech therapists to provide extremely functional treatments geared toward maximum independence.

Private Pay Nursing/Aide Services

Private Pay Nursing/Aide Services provide patient home visits when not covered by Medicare or other insurances. A home health aide can assist with light house-keeping, meal preparation, and companion services; and a nurse can provide medication assistance, continued education, and disease monitoring. Care is usually provided in blocks of time and paid directly by the patient or family members. Our staff is willing and able to provide what the patient needs when the patient needs the assistance.
